Print out a copy of the form. Fill out all necessary information, making sure you choose a division and day. Mail to 236 Beck Street   P7A 2K1

After April 1 phone your application to 343-0489. Do not mail it.

Incident Report

Although it is rare, incidents do occur that are not acceptable at Volunteer Pool Soccer Club.  We would like to know about them and have a chance to deal with them. Print a copy of the Incident Report or get one from the coach or Field Coordinator. Field Coordinators are our contact at each field. They wear a red jacket with "Field Coordinator" written on the back. Give your completed Incident Report to the Field Coordinator and they will bring it to the Executive. We treat all concerns seriously.  We do not want to see anything happen that will affect the enjoyment of the children in our club.
